Durham NCCARE360
Description and Activities
The Durham NCCARE360 Taskforce is now a part of the Access to Care committee. The Taskforce moved into Access to Care to ensure that all aspects of NCCARE360 are being included in The Partnership for a Healthy Durham's work. Representation in all of our committees will provide more input from coalition members.
The goals of this focused work still are to support the success of NCCARE360 in meeting resource needs in the community by:
- Engaging partners to send and receive referrals
- Identifying challenges and working towards higher utilization of platform
- Assessing how well platform meets needs of clients and partners
- Highlighting identified resource needs to decision-makers

What is NCCARE360?
NCCARE360 is the first statewide network that unites health care and human services organizations with a shared technology that enables a coordinated, approach for delivering care in North Carolina. NCCARE360 helps providers electronically connect those with identified needs to community resources and allow for feedback and follow up.
